You can provide feedback on your experience of our mental health services.
The "Your Experience of Service" (YES) survey is used across NSW to obtain feedback about what mental health services are doing well and what can be improved. Each mental health team/unit has it’s own unique service code, which ensures that the information provided is linked to the relevant part of the service.
For the past 2 years, a paper survey form has been used to collect feedback. Now you can complete the survey electronically on your computer, tablet or smartphone.
To complete the survey, it is essential that you have the correct service code for the part of the service that you want to provide feedback on. To obtain the code you can speak to a staff member from that team/unit or you can call 4734 1912. You can ask for a paper survey, or for a card with instructions to complete it online. All information collected is anonymous.
