Referral Criteria
-
18 years and above
-
Patients transitioning from 16 years old to 18 years old, in collaboration with a Paediatric Pain Service
-
No active substance abuse
-
Ongoing pain more than 3 months
-
Significant pain condition impacting on physical and psychological health
-
Patients wanting a rehab approach
How to Refer:
GP is advised to address the referral letter to one of the Specialist or to the Pain Clinic.
The specialist available in this department are:
- Dr Suyin Tan
- Dr Sushama Deshpande
- Dr Tai-Tak Wan
GP to complete ‘ Referral Guide to Adult and Paediatric Chronic Pain Services and Fax completed form to Nepean Pain Office together with the Pain Management Adult Referral Questionnaire completed by the patient.
Once the above forms are received, the pain questionnaire is triaged and patient will be offered to attend an introductory session, a pain programme and/or attend a pain clinic appointment. The patient will be notified by our secretary by mail or telephone regarding their appointment.

Living with Pain Programme - a 5 hour / 1 day programme (once a month - Tuesday and Thursday)
COPE Pain Management group program - a 5 hour / 6 week programme (3 times a year - Tuesday only)
