Referral Criteria

Diabetes Service

People with a positive diagnosis of Diabetes (ADS Criteria 2015) or Gestational Diabetes

The criteria for diagnosis of diabetes are now:

  • HbA1c > 6.5% (48 mmol/mol)
  • Fasting glucose > 7.0 mmol/L
  • Random glucose > 11.1 mmol/L
  • On a 75g oral glucose tolerance test: fasting glucose > 7.0 mmol/L or 2 hour glucose > 11.1 mmol/L

In an asymptomatic patient the test should be repeated for confirmation of the result and diagnosis. An abnormal result on two different diagnostic tests is also acceptable.

Diabetic High Risk Foot Service

Diabetes

Non-healing foot ulcer or wound

Suspected osteomyelitis or soft tissue infection of the foot

Suspected or diagnosed active Charcot Neuroarthropathy

Gangrene of the lower limb

 

How to Refer

A written referral is required. Referrals are accepted from Medical Practioners, Medical Specialists, NMBLHD service providers or other Ministry of Health services.

Referral Requirements and Pre Diagnosis

Diabetes

The referral agent can utilise a Diabetes Service Referral Form or provide information that includes:

  • Type of diabetes
  • Diagnostic values (if new diagnosis) or duration of diabetes
  • Most recent pathology
  • Diabetes treatment modality
  • relevant medical history and medications list
  • Current problem
  • Patient contact details
  • Referral contact details

High Risk Foot Service

GP to write detailed letter addressed to Dr Anna Zheng including:

  • Current list of medications
  • Current medical history
  • Latest pathology results (FBS,EUC, HbA1c, fasting lipids)
  • Imaging (X-rays, CT, MRI, arterial studies) if performed
